2nd: ₱1,000 FineReckless driving, DUI, no license, fake vehicle sticker, over-speeding.
1st: ₱2,000 + SanctionShouting indoors, loitering, sitting on railings, vulgar language.
Minor OffenseAny deliberate damage to university property, including graffiti.
1st: SuspensionKissing, necking, petting, sitting on a person's lap in campus areas.
1st: Major OffensePhysical, verbal, and cyberbullying. Includes unjust vexation and discrimination.
Sanction 2–4Possession: up to Sanction 3. Selling prohibited drugs: Exclusion (Sanction 5).
Exclusion RiskCheating, plagiarism, forging documents, false statements during investigation.
1st: Long SuspensionMembership in fraternities or sororities. Any act prohibited under RA 8049.
Exclusion (Sanction 5)Minor offenses aren't truly "minor" if they keep happening. Know the escalation ladder.
1st violation. Verbal advice given alongside the written warning.
2nd time. Formally recorded in your discipline file.
3rd time. One step away from full escalation.
4th violation. Formal investigation by the SWAFO Director begins.
SWAFO's procedures are built on due process. Every student in a formal investigation is entitled to these protections.
You must be formally notified of the charges against you before any hearing proceeds.
Submit a written statement and evidence to support your position at any hearing.
A parent, guardian, or adviser may assist you. Their absence does not invalidate proceedings.
Disagree with the decision? You have 5 school days to file an appeal to the University Discipline Board.
The Formation Program is SWAFO's official rehabilitation track for students who have served a major offense sanction. It's not a punishment — it's a path forward.
Students who complete all sessions and have finished serving their sanction become eligible for a Certificate of Good Moral Character — required for many scholarships, graduate programs, and government employment.
