Habitual commission of any minor offense of the same nature automatically escalates to a Major Offense, regardless of category.
Due Process

You have rights in this process.

SWAFO's procedures are built on due process. Every student in a formal investigation is entitled to these protections.

Right to Be Informed

You must be formally notified of the charges against you before any hearing proceeds.

Right to Present Your Side

Submit a written statement and evidence to support your position at any hearing.

Right to Counsel

A parent, guardian, or adviser may assist you. Their absence does not invalidate proceedings.

Right to Appeal

Disagree with the decision? You have 5 school days to file an appeal to the University Discipline Board.

Rehabilitation

A second chance is built into the system.

The Formation Program is SWAFO's official rehabilitation track for students who have served a major offense sanction. It's not a punishment — it's a path forward.

Students who complete all sessions and have finished serving their sanction become eligible for a Certificate of Good Moral Character — required for many scholarships, graduate programs, and government employment.

SWAFO is the disciplinary arm of DLSU-D — but its mandate goes far beyond issuing tickets. The office fosters the full welfare and formation of every student, promoting peace, order, and cultural sensitivity across the Lasallian community.

Enforce student discipline, rules, and regulations as stipulated in the Student Handbook
Maintain and promote peace and order across all university premises
Conduct formal investigations for major offenses and prepare investigation reports
Process student clearances before graduation, transfer, or other academic requests
Conduct the Formation Program and facilitate the Peer Assistance Program
Custodian of all lost and found items surrendered on campus

No anxiety, no confusion. Here's what students ask most about the SWAFO discipline process.

No. For traffic violations with fines, pay via QRPh anytime, then upload your receipt through GreenDesk. Your ticket moves from Unpaid → Pending, and a SWAFO admin will confirm payment, moving it to Paid. Your clearance won't be affected as long as it's resolved before you need one.
File an online appeal through GreenDesk within 5 school days of receiving the decision. Attach supporting evidence and write your statement. If your appeal has merit, the University Discipline Board (UDB) can commute, reverse, or affirm the original decision.
Your 3rd minor offense (4th violation total, counting from the initial written warning) automatically escalates to a Major Offense. SWAFO initiates a formal investigation and the SWAFO Director conducts a proper hearing. Sanctions range from Probation up to Long Suspension.
No. A Short Suspension is 3–5 school days; a Long Suspension is 6–12 days. However, at the SWAFO Director's discretion, you may choose an alternative: 1 day of suspension = 3 hours of community service or office work.
Yes. All student discipline records are protected under the Data Privacy Act of the Philippines. SWAFO does not disclose your records to unauthorized parties. GreenDesk stores your data securely — only you and authorized SWAFO accounts can access your records.
